Agreement for Career Counseling & Coaching
This agreement between DONNA SWEIDAN, as Coach, and $FIRSTNAME$ $LASTNAME$, as Client, includes 4 months (10 coaching hours)
Your investment for the coaching is $2750 or 4 monthly payments of $725, paid via Paypal. This agreement is valid for the next 4 months.
ENGAGEMENT: Coaching may address specific career, resume, social media, business and personal projects or general conditions in the Client’s life, profession or company. Coaching services may involve brainstorming, career exploration and assessment, career transition skills, networking and interview preparation, resume writing, personal branding, goal setting, personal empowerment, plans of action, and wellness. Due to the highly individualized and nuanced nature of our work, the Client's goals may evolve over time.
Assessment & Counseling
The purpose of the Assessment is to help you gain deeper insight into who you are, your strengths and value proposition. They are also designed to give you ideas of what direction to go in. Not all results will necessarily resonate with you. It is the job of the counselor to help you make sense of the results. The process of understanding the assessments take time and is different for each person.
Upon acceptance of this agreement, and payment you will receive all assessments discussed with your coach. Please complete the assessments / exercises that resonate for you at this juncture in time. If there is a reason you don't feel you need to complete a questionnaire, that's fine. You can discuss it with your coach.
As Client,
- I understand that there will be times when the deliverables will be intangible such as clarity, hope, confidence, insight, and motivation.
- I understand that career counseling/coaching is a comprehensive process that may involve all areas of my life, including work, finances, health, relationships, education and recreation. I acknowledge that deciding how to handle these issues and implement my choices is exclusively my responsibility.
- I commit to the coaching relationship by coming to the sessions prepared to participate fully with a topic I want to focus on and by being honest with my coach to design the coaching relationship that will work for me.
As your Coach,
- my promise to you is to be unconditionally constructive in the service of your goals. I ask your permission to be bold and forthright in coaching you. You can count on me to:
- Be honest and straightforward
- Ask powerful questions to deepen your learning
- Make requests to move you forward
- Hold you accountable for the actions you commit to
- Share resources where appropriate
- Be reasonable and flexible
CONFIDENTIALITY: As a matter of ethics, I uphold the Coach’s responsibility to maintain strict confidentiality about ALL personal information shared by the Client. The only exception is when the Client shares information that gives the counselor reasonable cause to believe there are threats of serious harm to the client him/herself or to others. I abide by the guidelines of the National Career Development Association. The Coach considers the current and future plans, business affairs, customer/client lists and personal and financial information of the Client to be proprietary. The Coach warrants that all proprietary information provided to Coach during the coaching process will be kept strictly confidential.
RECORDING: Your coaching sessions may at times be recorded. Such recordings are used for training and development purposes only and your anonymity will be preserved. Before sharing any of your session recordings with a coaching mentor, supervisor or assessor, I will always ask your written consent pertaining to the restricted release of a particular session recording first. You may also request a copy of any of your session recordings at anytime, and if a recording of that session has been made, it will be made accessible to you. You may also request that all your sessions be recorded, and all recordings made accessible to you at the end of each session. If you would prefer that none of your sessions are recorded, you may also advise me accordingly.
PAYMENT: In consideration of all services hereunder, Client agrees to pay the amount corresponding to his or her choice of initial coaching hours and payment plan (and, if Client requests any additional sessions after this initial term, the amounts as agreed in writing or via email between Client and Coach):
The amount corresponding to the plan elected is payable by Chase Quick Pay/Zelle, credit card or PayPal account and due in advance upon receipt of an invoice. Fees are based on 60-minute coaching sessions, which can be combined for longer sessions of minutes (2 hours, equivalent to two sessions) or 180 minutes (3 hours, equivalent to 3 sessions). Coaching time, once paid, is not refundable.
NATURE OF THE RELATIONSHIP: Throughout the coaching relationship, the Client can rely on the Coach to be honest and straightforward in asking questions and making requests. The Client understands that coaching is by permission and grants the Coach permission to coach. If Client believes the coaching is not producing the desired results, Client will communicate and work with the Coach to migrate the focus of coaching sessions toward such results.
This agreement will end in the case that either the Coach or Client decide that the relationship is not a good fit, or at the end of the coaching agreement as stated above. If this agreement is terminated prematurely, notice must be given within the first two weeks of the coaching agreement or before the third coaching session.
THE COUNSELING/ COACHING RELATIONSHIP IS LIMITED TO: career counseling and assessment, personal branding, career development coaching, resume development, LinkedIn and social media revamp and coaching, network development and assistance, interview coaching, salary negotiation coaching, small business development, and entrepreneurship.
AMENDMENTS, ENTIRE AGREEMENT: This Agreement may only be amended in writing signed by Client and Coach. This Agreement amends and supersedes all prior verbal and written agreements between Client and Coach.
RESCHEDULING: Client agrees to give twenty-four (24) hours advance notice of any need to reschedule a coaching session to 203-613-1049 or Donna@careerfolk.com.
Client enters into this Agreement with Coach with the understanding that Client is responsible for creating his/her own results and hereby releases the Coach and her agents from any liability for adverse actions or results experienced by the Client as a result of the coaching relationship.
By clicking "I Agree" below certifies that I have read this Client Agreement and agree to abide by all above policies and procedures. I also agree to hold the Career Coach and Careerfolk, LLC, harmless for any adverse situations created as a direct or indirect result of specific advice or referrals given by the Career Coach. I also agree to indemnify and hold harmless the Career Coach, and Careerfolk, LLC, from all claims, losses, expenses, fees including attorney fees, costs, and judgments that may be asserted as a result from the acts or omissions of the client. My signature acknowledges that I have printed out a copy of this Agreement.
